What to Expect During a Mobile Psychiatric Visit in Maryland

If you've never had a mobile psychiatric visit before, you probably have questions. That's normal. Most of the patients and families we work with across Baltimore, Waldorf, Landover, and the surrounding Maryland communities tell us the same thing on the first call: "I'm not sure how this works."

The short answer: a mobile psychiatric visit is a full clinical visit — the same care you would receive in an office — delivered in the place where you already feel safest. That might be your home, an assisted living apartment, or a skilled nursing facility. Here is exactly how it works, from the moment you reach out to the moment we follow up.

Step 1: Scheduling your visit

You can request a visit two ways: book online at nponcallmd.com/book-online or call our office at 240-898-1810. Either way, we will ask for a few non-clinical details first — name, date of birth, address, phone number, insurance information, and a preferred time window. We do not collect symptoms, diagnoses, or medication lists over an unsecured form. Anything clinical we handle by phone or in person to protect your privacy.

Most patients in Maryland are scheduled within a few business days. For nursing home and assisted living residents, we coordinate directly with the facility's nursing leadership so the visit fits into the resident's daily rhythm.

Step 2: Before the provider arrives

The day before your visit, you will receive a confirmation by phone or text. We ask you to have a few things ready if possible:

  • A current medication list, including over-the-counter supplements

  • Names and contact information for other treating clinicians (primary care, neurology, therapy)

  • Any recent hospital or emergency department discharge paperwork

If you are a caregiver, plan to be present for the introduction. Whether you stay for the full assessment is up to the patient.

Step 3: The visit itself

A board-certified psychiatric mental health nurse practitioner (PMHNP) arrives at your scheduled time. We bring everything we need: a clinical bag, secure electronic documentation, and the ability to send prescriptions electronically to the pharmacy of your choice.

A first visit is typically 45 to 60 minutes. It includes a full psychiatric history (mood, sleep, appetite, energy, focus, anxiety, trauma history when appropriate), a medical and medication review, a safety assessment, cognitive screening when indicated (especially for older adults), and a conversation with family or caregivers when invited. We move at the patient's pace. There is no waiting room, no traffic, no parking.

Step 4: The treatment plan

Before we leave, you will know what comes next. That usually includes a working diagnosis or differential, a medication plan that is started, adjusted, or held; recommended labs or coordination with primary care; referrals to therapy, support groups, or higher levels of care if needed; and a follow-up date.

Prescriptions are sent electronically the same day, including controlled substances where clinically appropriate and permitted under Maryland law.

Step 5: Follow-up

Most patients are seen again within two to four weeks for medication management, then on a cadence that fits the diagnosis — monthly, every six weeks, or quarterly. Between visits, you can reach our office at 240-898-1810. We respond to clinical questions within one business day.

What makes mobile psychiatric care different

Three things, in our experience. Comfort changes the assessment — patients tell us things in their living room they would never share in an exam room, which makes our evaluations more accurate. Family can be part of the visit — with the patient's consent, a spouse, adult child, or facility nurse can join, and that collaboration improves adherence and outcomes. Access is faster — mobile psychiatric care eliminates the two biggest reasons patients miss visits: transportation and waiting times.

A note on safety

If you or your loved one is in immediate danger or experiencing thoughts of suicide or harm, please do not wait for a scheduled visit. Call 911 or go to the nearest emergency department. You can also reach the 988 Suicide and Crisis Lifeline by calling or texting 988. NP On Call provides scheduled outpatient psychiatric care; we are not an emergency or crisis service.
