NP on Call provides psychiatric services to individuals in assisted living facilities, nursing homes, and their homes. For over three years our provider has been serving individuals in the community for medication management, psychotherapy, and substance abuse treatment. Clients with chronic mental illness have a high hospitalization rate and poor medication compliance; follow-up with outpatient services is poor for various reasons.

Our provider will make weekly visits to facilities to provide services to clients. We are competent in treating clients with depression, bipolar disorder, anxiety disorders, schizophrenia, mood disorders, and dementia. Services provided will be medication management and supportive therapy.

In case of psychiatric emergencies, clients will be safely transferred to the nearest hospital equipped to care for clients with needs for a higher level of psychiatric care.

Our goal at NP on Call is to reduce the 30-day readmission rate for psychiatric clients and improve outpatient care. One of our main focuses will be medication compliance and access to community resources.
